Yes, free plugin only supports one instance on same form OR same page.

You can opt for paid plugin of us.
Currently our paid plugin is not supporting Contact Form 7 5.4 or greater version.
We will be fixing that for paid version soon. https://codecanyon.net/item/multiline-files-upload-for-contact-form-7/20632083

Please check if zip extension is enabled on your server. It seems when more than one file is uploaded and plugin tries to zip it, it creates the problem.

We see this error log in response.

There is 500 internal server error.
It must be because of file upload permission or something?
Would you send error log of the wordpress to info(at)zluck(dot)com so we can debug futher.
