Correction, the repository is now located at: https://github.com/ZetaTwo/mm-forms-community as a part of getting the names consistent.

Also, when making pull requests. It would be really nice if you could separate different bugs/fixes into different pull requests since I have made a lot of modifications myself and it will be easier to merge that way.

I have published an update which addresses this issue. Unfortunately I only had time to repair the current solution and it looks a little strange with non images but it still should work.
