Hi I just discovered this problem. The plugin Cleantalk is up to date. But Wordfence is picking up fw_filesindex.php in wp-admin.

I thought that Cleantalk is supposed to delete it?

They say it is not an error. However we get downtime for a short time when it happens.

Also it is almost impossible to troubleshoot, if it is a plugin conflict, because it happens at random times, sometimes it doesn’t happen for 6 months.
