If you go to the bottom link you will see 2 form field boxes on that product page that say “Last Name :” and the other says “Country of Origin (if known) :”, that is what I need to accomplished for my new wordpress store, that’s why I was asking if your plugin supplies a shortcode for form fields. Hi, yes I have a client that sells mugs etc. with the last name on the mugs etc. I need to add a form field on the product page so the customer can add their last name.

Hi I have this figured this all out, thanks for the reply! The only problem I am having now is when I choose “no side bars” for a certain page I am getting double images at the bottom of the page, if I choose to use widgets all is alright. Does anyone know what I am doing wrong? I can’t link to the page because its on a wamp server ??
