As you mentioned, disabling all plugins and enabling them one by one helped resolve the problem. It seems the issue is related LiteSpeed Cache plugin. As noted earlier, the plugin works perfectly in our staging environment.

Could you kindly change your image and test it again? If the issue you’re encountering is image-related, this may resolve it. We have thoroughly tested the situation in our staging environment, and everything appears to be functioning as expected.

Thank you for sharing your form details. Your setup seems solid, with the quiz question, reCAPTCHA, and honeypot field, all of which are effective anti-spam measures.
However, I recommend increasing the time delay between form load and submission. A 6-second delay may still be too short to deter more advanced spam bots. Increasing the delay to 10-15 seconds should help reduce spam submissions.

I really appreciate your debugging efforts. It seems like a cache issue, and you’ve resolved it effectively. I also wanted to share an important update regarding the recent plugin update.
New Functionality Added:
We’ve added enhanced functionality to help prevent caching issues that may have caused the problem you’re experiencing. If the issue persists, please try the following:- Enable “Use Default Settings“: This should resolve the issue you mentioned by resetting to the default configuration.
- Use “Use Transient” Option: we recommend using the “Use Transient” function if you are experiencing the cache issue.
If anyone continues to experience difficulties, Don’t hesitate to reach out by opening a new support thread. We’re here to help!
