Anyone can now Google my URL can find this page indexed.

The form is integrated with PayPal (currently set to sandbox), so you can use the demo CC numbers here to complete submission: https://www.paypalobjects.com/en_AU/vhelp/paypalmanager_help/credit_card_numbers.htm

I’d certainly prefer not to setup an SMTP service just to send this second email. Let me know the results of your tests.

I’ve installed the mail logging plugin and can see it has been sending two emails. After several more tests the second email came delivered once out of 5 tries.

Would you still like to see the form? Is there another way to send the notifications without using wordpress as the mail sender?

That did the trick! It pops up over the header now just like I needed.

I was able to get it working by using the iFrame code, although I wasn’t able to get it directly from Facebook, nor was the full code ever shown for step 3. Very strange.

Thanks for the response. I added it using the method from the same link – using the javascript sdk (embedded in my header file) and placing <div class="fb-like"></div>in the text section of the custom content.
