This error has been caused by GP Premium plugin.

I hope this video can help you solve the issue.

Hi,

I have disabled all of my plugins except for the LifterLMS and LMS labs plugins.

The same results are happening. It is also doing the same thing as before. 1 for the 1st question, 2 for the 2nd, 3 for the 3rd etc…,

I have never seen the error you mentioned (send_origin_headers() ) before.
Also, I have performed this action using Google Chrome.

If you want me to do anything else please let me know.

the reason the question is not there is because when this happened it did not save. I had saved the previous questions before I noticed this.

This is still happening. I added several questions yesterday and the same thing is happening.

I managed to fix it. Although I replicated the site and found no problems, it seems there is an issue with the Easy Updates Manager plugin. Although i works in the replicated site, I noticed some strange behaviour when I tried to alter some update functions.
(I updated to 5.1, successfully), but the update manager plugin hung when trying to select option for updates to major and minor releases.
I disabled it, and the site works, but I also reenabled it and now the site is working as before.
I will keep an eye on it for now.

I am unable to login

My only id is [email protected] which does not let me in. I have tried to reset my password, but it gives me an invalid ID response.

This also appeared at the same time (roughly) on my Wordfence plugin.

I have several sites all running Wordfence, all on the same hosting company, on shared servers I believe, and all of the other plugins are working fine. I am also running LMS on another website hosted by the same company and that is working fine.

I have added the debug code to my wp-config.php file with the ‘true’ & ‘log’ what should I do to generate errors? I have created a debug.log file(there was not one there before).

1st error is ‘Internal server error’ when trying to create access plan (they have been reset)
