Hi,
I use the form short code to request [link-exchange-form], when people click submit. Where does the request go to? I don’t see anywhere to setup to go to my email address the notifications when a request is made?

Please advice, here is the link to submit a request in my website:

Also, when I click the submit button, a blank page loads up, when I look at the link, it is the same link where the form is loaded, with the only difference that this page is blank.
Can you explain how to setup up this?

I was able to fixed this error message, the error came up because the path for the file was incorrect. Once I fixed this error everything worked. The file name and the path is case sensitive.

I posted the above message, could someone with knowledge about this error post an answer for it.
