Unfortunately, it won’t be possible to create a form similar to the design you provided. You can have similar functionality but the design is a bit advanced and it won’t be possible to create a customized form like this using Fluent forms.

Also, Fluent forms don’t have integration with Woocommerce yet. Hopefully, it will be added in the future.

Unfortunately, it isn’t possible to create a blank input field within the radio field like the screenshot you provided. You have to take an input field separately and use conditional logic to display the field based on some conditions.
