The output is given twice in your page. Did you accidentally paste the [feedback] shortcode twice?
Also, there seems to be unnecessary paragraph tags inserted. Some shortcode plugin might cause this. Can you please share the list of plugins you are using?

I looked deep into your problem, and it seems this file: https://www.mantlesgroup.co.uk/wp-content/themes/wowway/js/plugins.min.js is causing all the trouble. The fact is that jQuery.tabs is defined twice and therefore breaking the script.

For now I’d suggest removing that file from your theme or simply edit form-class.php and remove the jquery.ui.tabs enqueue.

In future I will add more options to deal with such problems.

This is strange. I’ve checked in my localhost and it works fine… Can you tell me the exact ways to reproduce the problems?

It seems there is some jQuery conflict in your site. It is hard for me to say why it is exactly. What you need to do is, disable your other plugins one by one until the problem solves and then report here about the plugin which caused this problem. If the problem persists even after disabling all plugins, then the issue should be related to your theme.

You can either translate using the documentation available in wp codex. Or you can edit the /classes/form-class.php file and edit it yourself. (just search for the texts, Next, Previous, Submit etc…)

It has been fixed in version 1.0.2. It was a PHP 5.2 compat issue

The first two will be fixed/added in the next version. For the third, I don’t think there is any full proof system to block it. Even with unique email validation, people can use a different one. That is the problem with the online quiz system.

Willing to know what has caused the error! I think it was some infinite loop? As no matter how much memory I allocated it was just failing at the point where the breadcrumb is supposed to get loaded!

In the mean time I have downgraded to 1.2.3 which has solved the issue (using WP 3.4.1)
