I eventually traced the problem down to LastPass. It was filling that field. Have no idea how it got the string but after I deleted the account and reentered it, the problem was solved. I thought it was a server side issue since the problem was on all my computers and browsers but I had LastPass on all of them.

I used the Health Check plugin to debug. Went into debug mode with all plugins disabled and still had problem.
When I go to create a new page or post, I see the “Enter Title Here” text and then it is suddenly replaced with a title from a post created a while ago. Always the same title. Driving me crazy…

I’m using X theme but I switched to twenty twelve and got same problem.
I deleted Yoast SEO
I suspected it might be caused by Toolsets Post Type or Views but I deactivated both but no joy.

Still no resolution!
Update 1.2.6 didn’t resolve the issue for me either. Would be nice if the developer would at least acknowledge the problem and let us know that he is working on the problem. I don’t see reference to the problem on the development log. We can provide add’l info to help solve the problem if it’s needed.

Thought the Nextgen Gallery was the key, but alas, I deactivated Nextgen and still have the problem.

Same problem as original poster. I don’t have WordPress Meta installed. Tried IE 9 and Chrome.
