Description of the tags is wrong, rather it should be

%title fetch page/post title
%name fetches file name of the image
but the tags are working correctly.

If you use value %title for alt and title attributes than you will get page/post title/name and if you use %name than you will get the file name of the image that you upload say if you have uploaded an image having file name as yourimagefile.jpg then you will get yourimagefile as a value.

I think there is some confusion regarding image title, what image title you want there, if you want to show image name in the alt attribute than use %name instead of %title
