Just to make sure, you have tried different browsers and clearing caches, right?
It seems it’s not a bug generated by the plugin, rather some conflicts or misconfigurtions, what needs to be checked.

There can be a possibility that some custom script run when the registration form is triggered that is preventing the correct validation. But I can’t say for sure without checking.
Are these two checkbox generated from two different fields or the same field? Make sure you have configured the checkbox fields correctly.

The thing is, “Activate” user account is the equivalent of “Approving” user.
So, there is no need for other options like “Approve User”.

To avail admin approval or denial option, make sure to choose “Need admin approval” as the User Activation method from “User Meta >> Settings >> Registration”.

To see the files on the admin side, you can add the file upload field to “Extra fields in backend profile” from “User Meta >> Settings”.

Sorry! My mistake.
“Phone Number” field is not available on the lite version.
But you can use Custom Field with HTML 5 input type “Tel” or maybe assign a regex that suits your need.

You can try the “Phone Number” field.
But if you want to be more specific with rules and validation try “Custom Field” defining Regex for the unique number formatting that you are looking for.

Sorry that your experience with User Meta was not good enough.
Also thank you for the suggestion about Shared Fields.
Up until now, you can not actually create custom fields with the free version, unless you have installed the pro version and somehow deactivate the license. Although from the upcoming free version almost all the fields will be available in the free version too.

For the role problem, make sure you have configured the Role field correctly. Make sure to choose at least one role for “Allowed Roles” and select a “Default Role” from the allowed roles.

To add an extra field to the backend-profile, make the field as a shared field and then add that field from “User Meta >> Settings >> Backend Profile”. That should do the job.

It seems the email field is there in your provided URL.

Yes, registration with email only is possible.
In that case, username would also be the email address.

It might be a plugin conflict issue.
Please update User Meta Lite or User Meta Pro to the latest version.
If that does not solve your problem, please contact at support(at)user-meta.com preferably with temp admin credentials (if possible) so the issue can be checked based on your setup.
