Conect a form with a database

Can you share some additional details regarding the connection you have and the database? Which method or integration have you used in this case?

After exporting, please share the export through a Cloud Drive link or through a service such as https://pastebin.com – please use one of those as posting the form’s code here directly will most likely cause it to become unusable.

I’m only keep enabled the option to save in database but it doesnt works, what more can i do?
