The second checkbox is generated by the plugin named CommentLuv, while the last could be generated by one of the several “subscribe to comments” plugins (including the one integrated into WordPress Jetpack). I hope this might help you or anyone else who has this question.

I came to find out that this has to do with a core limitation of WordPress.
Interesting to find this thread here. I just experienced a similar case on a fresh WordPress install. I assumed it was due to a conflict with another plugin. After deactivating Admin Tree Page View and activating it again, the flyout menus returned. Could this be because of the order in which the plugins loaded based their activation?
I also noticed that they only appear when not viewing a page under the Pages menu.
Thanks for sharing. I hope to see this included in a future update of the plugin.

@mandr1: Actually, I’m going to have to say that you’re on to something. A colleague of mine and myself tested the plugin under IE9, FF10 + FF11a, Chrome 16 + 17b. Out of those the issue appeared under the Firefox browser tests. IE9 seemed to render the document fine, as well as Chrome. Then I came across your post and we tried being logged into Google Docs, where the PDF rendered fine under both versions of Firefox.
Thanks for releasing a quick update. It works flawlessly for me now.

While I don’t mind implemented functionality that is in high demand, what really sparks my interest in this plugin is the simplicity and minimalistic approach. I just hope it won’t escape it’s purpose and end up “bloated with useless functionality”.
Although, on a side note it would be handy to allow manually placing the ratings in templates via a function call.
