I used PHPmyadmin to dump the WP_USERS table to CSV, then imported that after formatting it according to the sample.

I am recently experiencing this problem with WP 3.9.1 and Ithemes Security 4.3.7. It was previously functioning fine, but now just reinstalling and activating the plugin causes the redirect loop. Suggestions? Are there database tables I need to clean up? Since I can’t access the plugin other than by deleting or renaming its folder via FTP.

Same issue with one of my sites – /wp-admin and /wp-login are ignored.

Only reference in .htaccess to wp-admin is:

# Rules to protect wp-includes
RewriteRule ^wp-admin/includes/ - [F]

Not using “hide backend”.

I am also experiencing this problem. WordPress is 3.7.1, PHP 5.3.

With your test version, the initial error text changes to
“SyntaxError: Unexpected token A An error occured. Please download log file and send to the plugin author.”

There are no “Settings” in the plugin. Can you tell me where this log file is? I only find out_log.php and .php~
