If you mean the bottom of each webpage, include the text in your footer.php file (or a text widget in your footer region). If you want it at the bottom of each post (ie, content area) then you can add the text in the single.php or page.php template file after the_content().

Awesome. That worked.
I put the code inside a Executable PHP widget. I did some more work and the form does display as part of index.php, but doesn’t seem to work inside the sidebar processing.

Yeah, I’ve discovered that same problem from searching many other forums.
With the package I had just created, there was nothing in the box on the link page. I used a prior package and the questions were there. From a usability perspective I think it would be far superior to just redirect to that landing page and have people fill in the info. It’s not obvious you have to click on that link to complete the process. I kept looking for an option on the settings panel for another page to complete the ticketing process.
Ditto this. Would like to be able to make a php function call rather than the shortcode for a template I’m making. Something like:

Yes, they are all there. Not sure how that’s relevant. There’s still only the “order” option available in the Attributes box, nothing about a parent wiki page.
Nope. The only attribute option is “Order”. And I can’t change the template of the entry either.

Doesn’t work. Like I said it somehow seems to be hard-coded, but I can’t find anything that’s making it display as grey.
