Since the latest update of the theme or is it the plugin that caused the problem do let us know.
For your form to be responsive you can add the following CSS:
Appearance > Customize > Additional CSS

 .everest-form{
margin: 10px;
}

}

It looks like one of your plugins is creating this issue. There doesn’t seem to be any problem with the theme.
I recommend, you deactivate your plugins one by one and see if it works.
