I deleted the settings and the log appeared, but it is still not appearing on my plugin list. I’ve deleted, reinstalled and activated. nothing. (and I’ve had some users say they’re experiencing timeout errors when accessing the site, not sure if it’s related.)

It’s every required dropdown list.

Thanks Nick, I’m not exactly sure what to do with this gist. Do I just create a new page titled pageTitle.php and put it in the plugins folder?

I got it. I changed the line:

to:

Chrome in OSX, the only visible section is the last line. See screenshot.

When this form is submitted (on any page) it goes to the bottom of the form.

Ok Nick, I filled out your form, let me know when you find something out.

So the best option would be? Premium support?

I switched to the generic WP theme, I disabled every theme and I even made those page subpages to see if that would make a difference. In each case, the error still persists.

What issue could it be with the host? PHP version?
