Maybe a problem with the .htaccess file.

You are using CloudFlare, maybe its help to turn the “Development Mode” on. (No Caching)
CloudFlare -> your domain -> Caching -> Development Mode

You planning to use “WP_CONTENT_DIR” plugin as default?
Not only in the class-litespeed-cache-admin-rules.php and htaccess file?
