You are right. It was varnish cache and some wrong rules on .htaccess. Thanks for the quick revert.

Sorry for not getting back, Jeff. I didn’t find time to debug further. As I mentioned, I manually added the code in functions.php and left it like that

I manually added the code in functions.php for now since it was affecting a lot of pages on the production site. Let me recreate the issue on a staging site.

It looks like the issue is with In-Content Related posts widget (part of this plugin only). Especially when some related post has no featured image set. All these days it used to automatically pick an image as featured image. Now it’s going blank.

That’s right. It still loads from both places. I’ve no idea why.

Just to be clear, it’s the exact same files served from server and cdn.

And sorry, I don’t know which settings you are talking about.
