This is because you have the background overlay color white. Just click “clear” and the background image will work.
The background overlay color option is just above the background image option.

This might be because you are using perfmatters caching plugin. It might lazy load things until the user focuses on the screen.

Please make sure you are using the latest version of Quill Forms.
Then, if the error still persists. It is almost caused by another plugin that causes redirect or something like that. The error code is 302 error (not found)

We already use in our plugin to prevent combining our scripts but it seems that it doesn’t work well:
quillforms/includes/compatibility/cache/sg-optimize/class-sg-optimize-compatibility.php

Since we don’t have any siteground server, we will try our best but also any help would be appreciated!

We have investigate the problem.
The problem is caused by LiteSpeed cache plugin because you combine the js. If you have deactivated the plugin, the problem will be solved.
Also, if you can disable the plugin only on the forms pages only, that would solve the problem.
We will try to have more investigation for this to let Quill Forms ignore the js combining done by LiteSpeed cache.
