Removing the multiple option is not possible as nearly all customers will want several services at the same time

Set up in the usual way
Under the form tab, I put the cursor where I want the field to go. I click the type of field I want to add (dropdown menu) and fill in the form generator pop-up box. I then click Insert Tag. I then manually wrap it all up as follows (actual example):

    <p>Services Required (required)<br />
[select* menu-834 multiple include_blank "Proof-reading" "Editing" "Ghostwriting" "Cover design" "Formatting - eBook files" "Formatting - POD (print) files" "Uploading eBooks to sales sites" "Uploading print books to sales sites" "Website creation"] </p>

This relates to the website: https://www.thornislandpublishing.com … where you can see the problem under Request Quotation.

    Hope you can help further
    Thanks

I have been in touch with my host and their engineers will look into the matter on Monday(it’s the weekend here in Ireland already).
Thanks for all your help and advice.

Thank you very much for your reply.
Re “Sorry, this file type is not permitted for security reasons” I have put in a request for support from my hosting service provider and am awaiting a response.
Re “Unable to create directory wp-content/uploads/2015/11. Is its parent directory writable by the server?” I followed the link you gave me. As I had uploaded WP using FileZilla as my FTP client I followed the instructions under that heading and changed permissions on the index.php file to 666 and clicked OK. I tried to upload the images again but got the same message. When I checked back to the index.php file I discovered that the 666 had reverted back to xxx, the numeric value when I first opened it. I tried changing to 666 and clicking OK several times but each time I reopened the file the permissions had reverted to xxx. Any idea as to what is going wrong?
    Much obliged for your help
