I’m having the same problem. Have WP 4.5.3 installed and no additional plugins added. Theme is Desk Mess Mirrored 2.5. Everything is updated. on 23 June all was working as usual and editing posts showed images as I worked. However, today when I entered site to edit and post, when I work in draft all I see is text and the images are placed as code only. When I preview the site the images are visible. I cannot find a way to change my work site to show visual mode to see images as I work. What gives to the change in visual/text mode?

Same problem and after struggling for two hours, trying all the suggestions, I instead selected “Add an image”. The two photos what had uploaded 0 x 0 at least 10 times trying everything else, uploaded perfectly. I was able to choose thumbnail and they show correctly in my gallery. Seems too simple but at least this finally worked for me.
