If I remember correctly, you need to edit the page that the form is on. Go to the Text Editor (as opposed to Visual) and remove the offending line. No idea how it got there in the first place!

Can I do that through ‘Contact’, do I edit the page itself, or do I have to get access to the html to remove it?
