I “solved” the issue by disabling the W3 Total Cache plugin caching options.

You have to manually remove the items from the menu. If someone has the link, though, it should be possible to access those unpublished (draft) pages anyway.
My solution was to change the post_type in the DB to something like “unpublished-page”.
So they won’t show up anywhere in the website.

Did you take a look at the screenshots?It’s more something like building an external admin panel that calls WordPress methods/functions to publish or edit posts.
Still I don’t know if this could be done as it sounds like using WordPress without its backend.

It’s probably some plugin you are using. Try disabling all of them and see if the bug persists.
I had the same issue but not on all posts and I couldn’t exactly tell what was triggering the bug.
I’m using the Hybrid 0.6 Theme and WordPress 2.8.3 but the issue presented itself with any theme.
I’ve now deleted the Digg This plugin. Solved for me.
