In the style.css of the custom PDF template directory I can see that body has css rule:

background: #fff;

Is it done by just changing this to:
background-image: url(“https://…”);

I’m using BeTheme and the plugin is not working correctly on my website I see now.

The Setup page misses the structured data part, also I’m not seeing the Shortcodes and Reviews tab.

I’m using version 2.14 of the plugin and WordPress 5.3.2. I updated the plugin to 3.0, but it doesn’t make a difference.

Have you heard about this before?

What do you mean with setting the Business Type?

Do you have a solution? It’s a big problem if you can’t submit on a iPhone

Having the same issue! Love Caldera Forms, but this forces me to use another plugin. Still looking for a sollution.
