It’s not a plugin issue, it’s due to your theme CSS overwriting the plugin CSS styling.
You can simply paste this CSS code. It will fix this issue.
You can paste this code from customizer settings Additional CSS.
Appearance > Customizer > Additional Css

.country-select input, .intl-tel-input input {
padding-right: 6px !important;
padding-left: 52px !important;
}

Regarding your CSS issue, kindly paste this code in additional CSS section from your admin panel. Theme > Appearance > customizer.

Regarding the auto country selected. I think you didn’t enable. Because I check your website source code.
It might be IP tracking code not working on your server.

The plugin is using the IP tracking script for auto country and dial code selection. Kindly check your browser console tab. if there any issue regarding fail the IP tracking call. So select dropdown will show the US and default selected value.

1. Regarding the form validation. I will try to fix this in next version. Which is coming soon ??

2. If you want auto dial code select. You can enable IP location feature. Please check did you selected US as default country from plugin setting or might be IP address code is not working on your server. The plugin has the condition,
a ) if any default country selected than it will show first.
b ) if no default country selected, and IP address feature enables and working on the server than auto select feature will work.
c ) if no default country and IP address feature not enabled or not working due to any reason. The first value from the dropdown will auto select.

Thank you for share screenshots. According to the console screenshot. It’s a server permission issue. Because script error showing 409 error code.
It server permission denied error.
I checked your website. There contact form 7 JS and CSS is showing this 409 error. My plugin based on contact form 7. So you need to check contact form 7 issue first.
Please try the plugin folder permission. It might be a broken plugin. You can delete it manually and upload manually than activate both plugins. The contact form 7 and country flag plugin.

Placeholder option is working. But fields are autofill. So placeholder is not visible. You can disable country dial code and the placeholder for phone number. It will visible there.

I am working on an empty dropdown placeholder for country dropdown.
