On instruction 5 you say the /init.php/0/ should be changed to 0 or 1 etc but I dont see this file.

I get this error also /blog/wp-content/plugins/vvvi-0.1/vvvi.php on line 61

I just tried your plugin and followed your instruction.

I get an error /blog/wp-content/plugins/vvvi-0.1/vvvi.php on line 61 and I noticed there is no /0/ after the init.php file??

Any ideas?
