You can display any form input values on a summary page by adding the field name.

Example:

[your-email] [your-phone]

depends on what name you are using in your fields

I go through your site and it seems like there is some error in your form. Some fields are not visible properly and the form submission is not working as well, When I try to submit the form it says “The email is required” but there is no email field appears for me.

can you please check your form fields?
