mk1623, sorry I don’t know the answer to this but I’m experiencing the same thing. Only the “Open All” “Close All” links and title show up but no category links. The strange thing is the plugin code is in the page source but it won’t show up on the page. I’m watching this post closely as I need an answer to this as well.
