Because your .csv is propably wrong separator. Use Excel is better for generate .csv.

Essayer de désinstaller le plugin complètement, supprimé tous les fichiers du plugin et re-installer le de nouveau.

Google Maps JavaScript API multiple times on this page. This may cause unexpected errors. Check disabled all other plugin and try again. I think is a conflit with other plugin. If the problem persists, tell me.

Because you use obsolete php version. Map Multi Marker work only with php version superior or equal to 5.6

I advise you to update the version of your php.
