I also experienced the same problem, After the upgrade version 4.1, visual text editor does not work so when I make a new post and edit old posts visual text editor does not work.

I also have to disable all plugins even change the theme into Twenty Fifteen, but the visual text editor still does not work.

I get the same problem. Hope this can be fixed.
I use the plugin: bootstrap-shortcodes.2.1, the icon does not show up in the text editor.

You can right-click in the browser (Mozilla), select inspect element and select inspector and then select element with mouse. seen on the right in the rules
