Hi sorry for slow reply. So I’ve attached some screenshots for an example. The first field is called “are you a:” and is a radio button with options “company” or “individual”. The next field is check box options called “which services do you require” and is conditional on “company” being selected. The next field is a number input box called “number of employees” and is conditional on “payroll” being selected in the previous field. I’ve attached screenshots of the settings of each field, a view of the whole form, and the email content with individual fields specified. I tried it with [all fields] and it didn’t work either. When the form is submitted the results of the “are you a:” and “which services do you require” fields are sent in the email but the “number of employees” is not.
