Are you using ProjectManager from localhost? Are you using XAMPP?
I have the same problem but only on my localhost (using XAMPP). Don’t know why. In my online host, the “add form fields” function works like a charm!
One more thing, be sure that you are not using IE. Use Firefox instead.
