I have the same problem. It’s annoying to chmod the uploaded files and the thumbnails by hand. FTP ist the only method to get the files uploaded correctly.

I had also the same error – every day. The repair-command didn’t fix the problem for ever.

So I tried this way:

1) Export the table wp_comments into a dump
2) Delete the table wp_comments
3) Import the previously saved dump

Now it seems to work. If the error occures again I will inform you.
