Another plugin can cause this. Did you try disabling tracking wordpress core activity?

Have noticed this from quite a long time. Saw this under different scenarios. Probably a bug.

I believe this will be available in the meta data in the DB. Not sure how can it be bought to wp admin.

You can exclude STREAM tracking some actions. That way you can prevent receiving emails for actions which are irrelevant.

An earlier version of STREAm had this feature but somehow majority disliked it and it was taken down.
