Put a text widget in your sidebar/header/footer/whatever, and paste the code in there. Does not work on WP.com hosted blogs though.

Make sure your adsense block will fit in the space you want it to. In other words dont put a 200×600 skyscraper ad in your 800×60 header!

I tried updating the password in phpadmin but it continues to do the same thing. No login, no error message, just refreshes the page.
