Update:
As it turns out the content from YouTube loads sometimes. After pressing update 10-20 times the videos gets displayed. Just bought the plugin to be able to use the Caching option. This is a ok workaround, but the problem is still there.

I have the same problem.
Mine started after updating to latest wp version or when i updated php5 to latest version. (Did both updates without checking the map function). Before that everything worked fine.

I have tried deactivating the plug in and delete all data, no luck.
I get this from apache/error.log each time i Activate the plugin:

I have optimized the database in an attemt to get rid of the duplicates as the errormessage suggests, lo luck.

I need to fix this problem, this plugin is awsome.
