I’ve deactivated Wordfence and cleared the cache everywhere and the problem persists. For now I’ve switched my lost password page and the following steps to another registration plugin that has also always been on my site which works fine after some tweaking to urls etc. so I have some usability back for my users (although the UI isn’t as good for the other plugin as yours). I will try deactivating all other plugins etc to see if there is a conflict elsewhere when I get the time, although I have not added any new plugins to the site in months.
The plugin has been closed again on the WordPress plugin page (this is now the third time I believe) and Wordfence is showing a critical error that “it has unpatched security issues and may have compatibility problems with the current version of WordPress”. Please clarify?

[my_calendar category=”3,4,5,7,8,9″ format=”calendar” time=”month” above=”nav,jump,” below=”none”]
If I remove “above..” and “below..” it does render the calendar, but not a single month view as it did. Can you advise what I should use to show a single month with no categories below
