Thanks for the support, it looks like the issues are not because of the plugin, I became convinced it has to do with my hosting provider. Maybe I will add the solution here for posterity, but you guys you can just ignore this.

Looks like we are talking about different things. I have nothing to do with Gutenberg or anything related to WordPress page editing I can add images to any of the pages there. I simply have the question pots, I need to add a new question to the existing list and when I add an image to the question it fails.
So I am using the editing field provided by the plugin.

This is the URL when I edit one of the questions:

Both of them have the same problem.
I am using the block editor I simply hit the “Add Media” button and copy the image link. It adds the image, I can see it, no problem. Once I click on save it gives me the error.

If I use the text editor I click on IMG button and I paste the link to the image, this adds to the content:
<img src="directPathToImage" />, when I save…error.
And then on save it gives the error again. However, if I switch from block to text and in reverse, the image is there. It only fails on save

I must add:
I have only one plugin, Quiz Tool Lite. There is nothing to deactivate
The images work fine in wordpress whenever I add them to any post or page
I checked the file permissions and recreated the .htaccess file
