I am having the same issue. When I upgraded the plugin to the latest version, it required me to do a sync of the providers list. That removed ALL of my providers and no matter how many times I sync the list again, they do not come back. i have tried deleting and reinstalling the new plugin with no success. PLEASE HELP.
I have tried resetting the DB and the providers list is not being rebuilt.
