I found the issue. It was disabled in the genesis setting.

It took me damn 2 days to figure out this.

Which hosting are you using?

My Issue got resolved as I bought premium plugin (Subscribers Magnet) for this task.

PS: Your code was also working fine later. I am using CDN on my blog so changes were not reflected at that time.

I don’t like Jetpack because it slow down blog very much.. You can use other popular Captcha plugin available out there.
