After doing some more research on your issue, I think all the options you are looking for have been moved to the “Notifications” tab in Ninja Forms. You can find more information on this in Ninja Forms documentation which is located here: https://docs.ninjaforms.com/article/36-notifications

After consulting a friend who is a bit more well versed in Ninja Forms and wordpress about your issue, it seems that caching plugins often cause this type of behavior. Do you have a caching plugin installed? If so, try disabling that plug in and see if the error continues.

Once again, I am not affiliated with Ninja Forms. I am just trying to help answer questions.
After consulting a friend who is a bit more well versed in Ninja Forms and wordpress about your issue. He informed that we might need a System status and a CSV export. A major problem is that Excel does not handle unicode well and a lot of time Ninja Forms is working fine, but the program that is being used to open the CSV has no idea how to interpret all of that data.

As of right now Ninja Forms does not have right to left support. This is likely why you are getting gibberish.

If you go to the “field settings” tab inside the “Template Field” box you should see a box called “submit”. Add a “submit” field to your form and a submit button will appear on your form.

Try backing up your theme and switching to the “twenty fifteen” default theme. If that doesn’t work try deactivating other plugins, sometimes plugins do not work well together.

In the field settings tab, when you create a new field on your form there will be a box next to the “required” setting called “Custom CSS”. Add the following code to the “Custom CSS” box ‘‘ This lets ninja forms know to make the label bold.
