Lol, had the same problem and need bit time to solve it. But indeed it is very easy. Create eyour checkbox, radio, whatever and (this is very important) use the name which is suggested from tag. So here is a example from my page (I use a menu-field):

code in formfield (customer can choose a date):
[select menu-649 include_blank “17./18. November 2012” “01./02. Dezember 2012” “19./20. Januar 2012”]

and in Mailfield I use the code:
Gewünschter Termin: [menu-649]

Thats it. Just use the same menu description. It works well.

below codes I use in my messagetext field (not in the formfield). After I have insert this, it works.

Vor und Nachname: [your-name]
Strasse: [Strasse]
Telefon: [Telefon]
eMail-Adresse:[your-email]
Nachricht: [your-message]

For example
In form field I use: [text* your-name] </p>
and in Mailfield I use: [your-name]

do I understand it correct that your form is as you want it but the informations are not in your mail? I had the same problem and note too late that you have to copy the line below also. For example you generate a Tag for a textfield and get the code you have to copy in left (into the form). Furthermore you get a second code below. This one you have to copy into the Mailform below into the message field. Do you understand what I mean? After saving the form you can check it and it should work. In the messagebox you have to copy all second lines of you Tags. Please try and come back. If not, I can send you screenprint how to do.
