As noted above, I have already run the Sucuri and unmaskparasites scans, both of which came back clean.
The ottopress article tells me what hacked code looks like, but doesn’t tell me how to resolve the problem.
The wordpress articles tell me to do pretty much everything I listed above as having done already, aside from a complete deletion and rebuild of the site. So, by eliminating everything I’ve already done, am I to surmise that the latter is the suggested solution?
And Questas, I’ve seen you recommend that plugin in several threads. It’s not going to solve a problem that already exists, but I will look into it to prevent further problems.
If he has a WordPress site as well, then the PrettyLink plugin would do the job. Have him install it, then generate a PrettyLink to your page and update all instances where he has a link to your page on his site. PrettyLink has a counter showing how many people click on each custom link.

If so, then that software is adding the style elements. Try pasting into the text editor instead, to strip those elements, then styling the post how you want it.

Our other four sites running 3.5, on the same VPS, are not experiencing any missed posts. I’m having a similar problem. I want my images (not thumbs) to show up at 400×300 and have set the NG options to reflect that, but they’re still being resized to 800×600. It ain’t pretty!
