Hey,
You need to put this code to your theme file, anywhere you want. For example open your theme file header.php, and paste this code there, and save it. After save look, at your wordpress blog, and you will see icons on header.

Plugin is fixed (1.1.3.1 version).
Sorry about that, just have deleted </div> tag by accident and missed it.
