Well … any URL that has “?post_type=bnfw_notification” in it and the pages linked on those pages. Simply the whole plugin backend uses the site language instead of the current user profile language.

1. We most certainly are fully ok to pay for such kind of features. We’re currently using the free version as we’re more or less evaluating different options yet, but probably will go with the paid WP Cerber anyway, when we go to production of our project, as it is our main contender for now.
2. I’ve checked the documentation (when I was looking if TOTP is supported in the paid version) and the options there are certainly very nice, but can’t compensate for the lack of TOTP. It is de facto standard nowadays and I strongly believe you should offer it (along with 10 recovery codes). You’re right that many people prefer email (or can’t handle TOTP), I know this from my own experience, that’s why it should be configurable – which one primary and which one secondary (if at all), but this can’t be a reason to not offering it as an option.
3. Frontend settings – this is good news.
4. 3rd party forms- now this is certainly “bells and whistles”, so I just mentioned it as nice to have. The reasoning behind it – would allow fully custom login experience, that’s all.Stay safe and keep up the good work.

Trialing the (inexplicably pricey) paid version. Getting the same as @iqcomputing with plugin version 2.2.1 and WP 6.0. This is when email is used as backup method. TOTP and backup codes working fine, but email backup method gives after the code is submitted :

Have in mind I’m using this in conjunction with “Theme My login” plugin (and it works for TOTP and Backup codes, as I mentioned), although very … very unfortunately “WP 2FA” does no offer any kind of integration with it (or other login types like ajax logins for that matter).
Thanks for the workaround, but would be really nice if you consider it. We’re multinational team and some prefer the UI in Dutch, others in English, so the real solution would be to follow the profile language indeed. I also think it is very logical to be that way as this profile setting is dedicated especially for this more or less.

Looking forward for the fix. As of the “premium” comments – no need of discussion, just wanted to share a regular consumer feelings about it. It’s totally up to you (of course) to consider if this is of any importance or not. If you spend some time of really evaluating my honest opinion would be more than enough.
