I had exactly the same problem as you have. After a few hours of debugging I’ve find out why this great plugin is behaving so strange on my server.

The cause of my problem was the Mirage 2 function from cloudflare. (Lazy_Load_function).

Are you using cloudflare Mirage function or any kind of Lazy Load / Caching plugins?
