Figured it out. Posting solution here in case anyone else needs it. This is the correct usage:
%%cf__company_name%% %%sep%% %%title%% %%page%%

Thank you Dave! The issue on my site was not what Hugh suggested but a host related issue like you mentioned. You saved me!
I tried that and it wasn’t it. The issue ended up being how my host (FlyWheel) was handing specific types of nginx requests. They were able to change how their hosting works and the issue was fixed.
That could our problem as well, except we’re on FlyWheel. The error we get when visiting https://nggcgisnek-flywheel.netdna-ssl.com/podcast-download/15963/sunday-sermon-august-16-2015.mp3?ref=download is “404 Not Found – nginx”
Thanks for the tip. Contacting my host now.
We’re having the same issue. For example:
On the back end the Audio File and Enclosure URL references point to: https://hinsdale.church/wp-content/uploads/2015/08/sunday-sermon-august-16-2015.mp3
BUT, when viewed on the front end the URL is changed to https://hinsdale.church/podcast-download/15963/sunday-sermon-august-16-2015.mp3 and leads to no where.
I’ve tried changing/updating the Permalinks like the documentation suggests but that didn’t fix the issue.
